Parents and Guardians of Glen Waverley Primary School may expect to receive a phone call from sick-bay from time-to-time. We may notify Parents/Guardians of an illness or noteworthy injury that has occurred. Rest assured, we will competently identify any medical and traumatic emergencies that may occur during school hours.
For the Parents/Guardians of children with anaphylaxis, allergies, asthma, diabetes, or epilepsy. You are required to provide an action plan for your child completed by your Doctor. Action plans detail the instructions for school staff to follow in the event of a medical emergency. Action plans help us to provide a safe environment for your child and ensure their health and wellbeing during school hours. Moreover, it is an Education Department directive that schools are provided with new medical action plan annually.
Medical action plan templates are available via the links below and at the school office. Once completed by your doctor, please return to the school office. Wherever possible, it is encouraged that medication is administered outside of school hours. If your child has a medical condition that requires a prescription medication to be administered during school hours, you must complete a Medication Authority form. All medication must be given to the school office at the commencement of the day, in its original packaging. No medication (apart from Ventolin) is to be kept in school bags or the classroom.
